Chessa Campbell, who started at Pennyroyal Hospice as a social work intern, has worked closely with Big Bend Hospice in Tallahassee, Florida to get the program launched locally.
The program helps people with serious illnesses navigate the healthcare world.
Campbell adds it’s easy to start the conversation about Transitions.
Martin adds there will be opportunities to volunteer with the Transitions program after COVID-19 is gone.
Pennyroyal Hospice serves residents in Caldwell, Christian, Lyon, Todd, and Trigg counties.
